Welcome Pastor Jo

Staff Parrish Relations Committee is compiling a welcome book of information provided by UUMC members about themselves. We will give this book to Pastor Jo before her arrival so she can get to know us.

Please download this form and either fill it out on the computer or print it out and complete it by hand. You can drop it off at church, mail it to the church, or e-mail it to Vicki at docvj89@gmail.com.

We would really like to include a picture with the form. You can paste the picture in or just include it with what you return as an attachment. If you would like to use your picture from the last UUMC picture directory, just let Vicki know.

We need to get this back from you by June 17 so we have time too assemble the book and get it to Pastor Jo.
